What began as prosthetically driven planning for traditional axial implants has progressed into highly sophisticated digital workflows that now support the precise placement of zygomatic and pterygoid implants. This lecture explores the clinical and technological evolution of guided surgery, highlighting the transition from static guides for standard implants to advanced digital protocols capable of managing complex anatomical challenges. Emphasis will be placed on digital planning, CBCT integration, prosthetically driven workflows, and the biomechanical considerations required for long implants in remote anchorage sites such as the zygoma and pterygoid region. Through clinical cases and step-by-step workflow analysis, participants will gain insight into how modern guided solutions enhance surgical accuracy, reduce invasiveness, improve safety, and enable immediate loading protocols – even in extreme atrophy cases. Severe maxillary atrophy represents a significant clinical challenge, primarily caused by irreversible bone loss following tooth extraction, sinus pneumatization, and periodontal disease. These conditions frequently result in reduced vertical and horizontal bone volume, poor bone density, increased interarch space, and limited possibilities for conventional implant placement, complicating prosthetic rehabilitation. In recent years, graftless treatment concepts have gained increasing attention as predictable alternatives to extensive bone augmentation procedures. Advanced implant techniques – such as pterygoid, zygomatic, nasal, and tilted implants—allow clinicians to utilize remote cortical anchorage to achieve primary stability even in severely resorbed maxillae. Despite these surgical innovations, prosthetically driven treatment planning remains the cornerstone of successful rehabilitation. Correct implant positioning must be guided by the planned prosthetic outcome in order to optimize esthetics, biomechanics, and long-term function. The use of multi-unit abutments enables correction of implant angulation; however, greater angulations may alter the position of the prosthetic screw access channel and influence the final prosthodontic design. A comprehensive understanding of maxillary anatomy, precise identification of cortical anchorage points, and meticulous prosthetic planning are essential for predictable outcomes.
